Job description

Location : Remote (must be available to respond to calls / texts within 2 hours during agreed-upon on-call schedule)

Hours : As-needed / on-call. Hours highly variable depending on patient needs. Weekdays 6 : 00 am - 9 : 00 pm PST. 2-3 weekdays each week.

Requirements

As a Health Navigator, you will serve as a vital link between patients and healthcare services, providing medical advice, guiding resource navigation, and assisting in urgent health situations. This role requires empathy, medical knowledge, and multitasking skills in a remote setting.

Responsibilities

  • Respond promptly to patient inquiries within set hours; callbacks or texts within 2 hours, no later than 24 hours
  • Provide health and medical advice via phone or text
  • Guide patients to appropriate healthcare resources within Sprout and publicly available services
  • Assist in urgent care directions during emergencies, supporting but not replacing emergency services
  • Help with appointment bookings when needed
  • Research or refer patients to specialists when outside scope
  • Proactively communicate availability changes
  • Conduct Medical Intake Form review calls with patients
