A healthcare provider in Halifax is seeking a Health Care Support Worker for a part-time position focusing on community mental health care for older adults. Due to retirement,we have a Part Time (22.5hrs p/w) Band 3 Community Mental Health Support Workervacancy. There is a requirement for thesuccessful candidate to work Monday to Wednesday.
We are aspecialist service that provide community mental health for older adults livingin Calderdale, West Yorkshire. Ourmission is to work collaboratively with service users and their families so toenable them to reach their full potential and live well in their community. Ourservice users live with a range of mental health conditions which can includeDementia.
We have astrong and well established Multi-Disciplinary Team (MDT) who work together todeliver person centred assessments, interventions, and support.

The main duties of the post include:
Independently andautonomously support, implement interventions and care for patients withattention to mental health/dementia, physical, emotional, social, and culturalissues.
To visit patients in theirhome.
Holda clinical caseload whilst liaising with the care co-ordinator.
Provide a joined upservice across health, social and voluntary care providers.
Be responsible for providing expertclinical advice and support to patients and their families.
Work in a collaborativemanner with others to ensure seamless service delivery.
The qualities we are looking for in potential candidatesare:
Established clinical backgroundin working with service users presenting with mental health issues.
Ideally to have experienceof working with older people living with dementia.
Experience of working autonomously.
Excellent communication, organisational and interpersonalskills.
A full comprehensive induction will be provided andaccess to further training options. The service places staff clinical andmanagement supervision, appraisal, support and professional development as ahigh priority.
A Full UK driving license and use of own car isessential.

The post holder will:
Under thesupervision/direction of registered health care professionals/associatepractitioner, within a delegated framework for agreed tasks, seek support asand when necessary, in the delivery of psychological and physical care to adefined group of people.
As part of a healthcareteam, provide the highest standards of safe and compassionate planned care topeople that maximises their recovery and rehabilitation potential.
Develop and maintainmeaningful therapeutic relationships with people. Adopt a polite, positive andempathetic approach, communicating effectively and informatively.
Demonstrate anunderstanding of the care and treatment programmes of a designated group ofservice users to enable you to instruct, advise & guide service users,family and carers.
To make the appropriate adjustments independently within ownlevel of competence and be able to communicate any changes to service users andregistered health care professionals.
Be required to concentratefor periods of time, depending upon the care intervention/care plan eg,following the observation policy, taking, recording of physical observationsetc, and responding to issues of concern appropriately. This may include areasof work requiring physical effort, with regard to risk, health & safety.
Exercise a degree ofindependence, initiative and judgement and consider different approaches whendelivering tasks. Assess situations, identify and resolve potential problems,within own levels of competence.
Ensure the careexperienced is positive and service user safety is maintained. Treat everyoneeg, service users, their relatives, visitors, colleagues etc, with respect,dignity, courtesy and in accordance with the Trusts Values.
Contribute to the careplanning process. Understand the principles underlying care planning eg,enabling the service user to reach an optimum level of functioning/independence/recovery.
Implement care withinwell-established policies, procedures, protocols, guidelines and establishedclinical care plans. Organise a range ofactivities eg, facilitate meaningful activities for individuals or smallgroups.
Apply knowledge of longerterm health conditions eg, Diabetes, Obesity, smoking cessation etc, providingadvice and support.
Undertake the physicalaspects of care eg, moving and handling of service users. Use items ofequipment in delivering care eg, hoists, wheelchairs etc. (as appropriate tothe role), ensuring appropriate instruction on their use has been received.
Assist in dealing withverbal/physical aggression using de-escalation techniques, this may include theneed for restraint and/or seclusion (as appropriate to the role).
Attend to the serviceusers personal care needs as required.
Participate in a flexiblerostering arrangement providing 24-hour/7 day care (as appropriate to the role).
