Health Care Coordinator - Courtyard Gardens (2025-8669)

About the Role

Under the supervision of the General Manager, organize 24-hour resident care in accordance with nursing standards and provincial regulations. Implement current health authority standards and ensure quality care.

Qualifications

  • Licensed Practical Nurse (LPN) in good standing.
  • Fluent in English, both written and verbal.
  • Strong leadership and organizational skills.
  • Proficient with Word, Excel, and healthcare management systems (e.g., MDS/Rai).
  • Minimum 1 year experience as an LPN in long-term care and 3 years in a leadership role.
  • Experience working within a union environment preferred.
  • Resident-focused with values aligned with Verve Senior Living.

Key Responsibilities

  1. Participate in Quality Improvement programs.
  2. Interview residents to assess needs and make service recommendations.
  3. Maintain accurate resident records and ensure medication safety.
  4. Develop and review care plans quarterly or as required.
  5. Conduct performance appraisals, recruit, and mentor staff.
  6. Manage staff scheduling and department budgets.
  7. Ensure compliance with health and safety legislation and infection control guidelines.
  8. Provide educational in-services for staff and residents.
  9. Contribute to leadership and operational improvements.
  10. Build relationships with residents, families, and community partners.
  11. Assist in marketing efforts and ensure mandatory training completion.
  12. Perform other duties as assigned by the General Manager.
