Position Overview

Reporting to the Director of Health & Wellness, the Health Care Aide provides personal care and performs selected tasks within their knowledge and skill, under the supervision of their supervisor, while adhering to regulatory and Extendicare's standards, policies, and procedures.

Key Responsibilities
  1. Adhere to the plan of care as determined by the supervisor;
  2. Facilitate residents' move-in and transfers where applicable;
  3. Observe and report on residents/clients care/service utilizing appropriate Extendicare forms or tools;
  4. Assist residents with personal care, hygiene, and activities of daily living;
  5. Observe and report changes in residents' condition to supervisor;
  6. Escalate medical emergencies to supervisor or emergency medical services as appropriate;
  7. Plan and prepare nutritious meals as required;
  8. Assist with ambulation, mobilization, transferring, and adaptive aids;
  9. Demonstrate understanding of resident needs and maximize abilities;
  10. Provide cognitive, social, emotional, and psychological support to residents and their families;
  11. Maintain resident information confidentiality;
  12. Respond to concerns and ensure appropriate action within your authority;
  13. Maintain a safe and clean environment, including residents' equipment and belongings;
  14. Observe and report safety concerns, implement interventions, and advocate for residents;
  15. Engage with other agencies involved in resident care as directed;
  16. Attend in-services to develop knowledge and skills;
  17. Work in a position of trust with vulnerable adults;
  18. Uphold the organization's values and ethics as per the Code of Conduct;
  19. Perform other duties as assigned.
Qualifications
  1. Graduate of Health Care Aide program or equivalent with minimum 500 hours of theory and practicum;
  2. Experience working with seniors in community health settings;
  3. Strong communication skills;
  4. Organizational and time management skills;
  5. Interpersonal and customer service skills;
  6. Proven ability to perform personal care duties;
  7. Current Basic Cardiac Life Support and First Aid certification recommended;
  8. Ability to perform tasks like lifting and transferring clients;
  9. Provide a recent (within six months) Vulnerable Sector Check or willingness to obtain one.
