Reporting to Community Care RN Case Managers, the Health Care Aide provides personal care and performs selected tasks within their knowledge and skill, under the supervision of their supervisor while adhering to regulatory standards, policies, and procedures.
Key Responsibilities:
- Adhere to the plan of care as determined by the supervisor.
- Facilitate residents' movement and transfers where applicable.
- Observe and report on residents/clients' care/services using appropriate forms or tools.
- Assist residents/clients with personal care, hygiene, and activities of daily living.
- Observe and report changes in residents/clients' conditions to the supervisor.
- Escalate medical emergencies to the supervisor or emergency medical services as appropriate.
- Plan and prepare nutritious meals as required.
- Assist with ambulation, mobilization, transferring, and use of adaptive aids.
- Understand resident/client needs and maximize their abilities.
- Provide cognitive, social, emotional, and psychological support to residents/clients and their families.
- Maintain confidentiality of resident/client information.
- Respond to resident/client/family concerns and ensure appropriate actions are taken or concerns communicated to the supervisor.
- Assist in maintaining a safe and clean environment, including residents'/clients' equipment and belongings.
- Observe and report safety concerns and implement interventions as directed by the supervisor, advocating on behalf of clients to ensure safety.
- Engage with other agencies involved in resident/client care as directed.
- Attend in-service training to develop knowledge and skills.
- Work in a position of trust with vulnerable adults.
- Uphold and promote the organization's values and philosophy, particularly regarding ethics, morality, and integrity as outlined in the Code of Conduct.
- Perform other duties as assigned.
Qualifications:
- Graduate of a Health Care Aide program or provincial equivalent with a minimum of 500 hours of theory and practicum.
- Experience working with seniors in community health services.
- Strong written and oral communication skills.
- Organizational and time management skills.
- Interpersonal and customer service skills.
- Proven knowledge of providing personal care duties.
- Current Basic Cardiac Life Support and First Aid certification recommended.
- Ability to perform necessary tasks such as lifting and transferring clients.
- Provision of a current (within six months) Vulnerable Sector Check, including a Criminal Background Check, or willingness to obtain one.
Wage Leveling Salary: $28.74 per hour
