POSITION SUMMARY:

The Health Care Aide provides direct client care and environmental support within established standards of care, policies, and procedures. They work to meet the changing needs of clients and assist them in reaching and maintaining health and independence under the direction of the team leader.

RESPONSIBILITIES:

Overview:

• Assist clients with activities of daily living, maintaining self-respect, dignity, and independence, encouraging self-care within their abilities, and integrating the resident Bill of Rights into care.
• Follow individualized care plans, provide feedback for care improvement, and report client changes to the team leader.
• Document care and observations according to policy, including in health records and flow sheets.
• Assist with personal hygiene, elimination, mobility, and other care activities as per care plans, ensuring safety and dignity.
• Monitor and report changes in client condition, record vital signs, and assist with medical equipment and procedures as directed.
• Maintain a safe environment, promote infection control, and ensure proper equipment use and storage.
• Assist with admissions, transfers, discharges, and post-death care, including valuables management.
• Support client participation in activities, organize leisure activities, and demonstrate safe medical equipment use.
• Perform other duties as assigned.
