Position Summary

The Health Care Aide provides direct client care and environmental support within established standards, policies, and procedures. They meet the changing needs of clients, assisting them in reaching and maintaining health and independence under the supervision of the team leader.

Responsibilities
  1. Assist clients in activities of daily living, promoting dignity and independence, integrating the Bill of Rights into care.
  2. Follow individualized care plans, provide feedback, and communicate client changes to the team leader.
  3. Document care and observations accurately according to policy.
  4. Support clients with personal hygiene, mobility, elimination, nutrition, and safety as per care plans.
  5. Monitor client condition, update records, and report changes.
  6. Assist with application of medical devices, safety measures, and medication safety principles.
  7. Participate in admissions, transfers, discharges, and post-mortem care.
  8. Maintain infection control practices and a clean environment.
  9. Perform other duties as assigned to ensure comprehensive client support.
