The Grief & Bereavement Coordinator, reporting to the Executive Director, Hospice, will work as a member of the inter-professional team, providing individualized and group-based support to community clients (caregivers and the dying), as well as the patients and families at AB Smith Homestead House as needed. The Coordinator addresses the emotional and psychological challenges of advanced illness, dying and bereavement by conducting ongoing grief assessments and delivering appropriate grief interventions. Through compassionate, evidence-informed care, the Coordinator offers anticipatory grief support and guides individuals through the evolving states of loss and adjustment.

Providence Care Hospice serves those who are impacted by a progressive life-limiting illness at an advanced stage. Our programs and services, address symptom burden experienced by the dying and their care partners at end-of-life and extend through bereavement. The staff and volunteers are part of the palliative care team and focus on building meaningful connections to support the well-being of the people we serve.

Skills & Qualifications Required

  • Current unrestricted registration with the Ontario professional college of discipline
  • Master’s Degree in Psychology, Social Work, Counselling or related field with a specialization in grief and bereavement.
  • Two years of experience in grief and bereavement, hospice, or mental health counselling. This includes individual support as well as facilitating evidence-based groups appropriate for the population served, including individuals with clinically significant grief reaction
  • Advanced education and training in grief and bereavement counselling; certification, preferred
  • Demonstrated knowledge of the stages of grief and bereavement and the ability to provide support at all points in the trajectory
  • Demonstrated ability to work sensitively with diverse populations
  • Experience implementing capacity enhancement and knowledge exchange activities to support the expansion of grief and bereavement expertise in the region
  • Demonstrated assessment and evidence-based group and educational interventions skills
  • Demonstrated ability to implement clinical guidelines/evidence-based practices
  • Demonstrated case co-ordination and consultation skills

Position Responsibilities

Clinical Support and Specialized Grief and Bereavement Counselling

  • Receives and triages referrals from internal and external sources; collaborates with Community Care Coordinator for those who do not require professional counselling services
  • Provides profession specific, evidence-based and time-limited interventions, including grief and bereavement counselling, in individual and group formats in alignment with palliative care principles.
  • Conducts assessments and develops tailored care plans based on the individuals’ needs; for bereavement clients this includes providing recommendations for recovery-oriented treatment plans to the client and their referral source.
  • Interventions shall be directed to enhance clients’ readiness, ability to achieve their desired goals, ability to manage their symptoms, and function in daily life.
  • Involves the service recipient in the scope and focus of the assessment in accordance with their expressed needs and desires
  • Facilitates ongoing evaluation and documentation of care outcomes to support the program enhancements and quality improvement
  • Provides case-based consultations and education to providers who contribute to the continuum of care
  • Identifies additional supports and educational needs of clients, making referrals to other community partners as appropriate

Program Development

  • Participates in the discovery and dissemination of new knowledge consistent with the organization’s academic role
  • Designs, implements and evaluates grief and bereavement programming that reflects best practices and community needs
  • Participates in the development of service protocols and service agreements with key internal and external partners
  • Maintains awareness of changes in social and health policies and the impact on the defined population

Interprofessional Collaboration

  • Provides topic specific education and support for learning and development that can lead to team solution finding, practice improvements, and capacity enhancement through human resource development

Community Collaboration

  • Establishes and maintains effective working relationships with local service providers and community stakeholders with a goal to optimize the provision of services within local/regional hospice program parameters, ensuring effective resource utilization
  • Participates on local, regional and provincial committees as appropriate
