Qualifications

Position Summary

Reporting to the Clinical Manager of Geriatrics, the Geriatrics Service Clinical Coordinator is an integral part of the Geriatrics Consult Service at the Brant Community Healthcare System. You work in collaboration with this team to provide expert clinical assessment and for making recommendations regarding the health care needs of the geriatric population across our hospital. Liaising with patients, families, and partners in the community and hospital, you will provide excellent, evidence-informed care to our geriatric population. You have the knowledge of community resources and are able to apply principles of patient and family centered care to your work and have skill in assessment, health promotion and health teaching. You have the ability to use critical thinking, problem-solving, and decision making skills and you possess excellent communication and interpersonal skills. You are able to work in a fast paced environment and effectively prioritize patient care needs.

Primary Responsibilities

  • In collaboration with the Geriatrician and Geriatrics Team, complete comprehensive assessments and identify the needs of elderly patients in hospital
  • Triages referrals with Geriatrician support
  • Act as a resource/coach to staff in the care and management of elderly patients with complex medical and social issues
  • Responsible for tracking and coordination of consults and the team’s priorities
  • Obtains geriatric specific history from patient, family or other source which will contribute to comprehensive geriatric assessment
  • Provides follow up on selected patients, upon direction by Geriatrician
  • Provides recommendation regarding specific aspects of geriatric care (non-pharmacological strategies for delirium prevention, continence)
  • Facilitate timely and safe discharge of elderly patients and to link them to community resources.
  • Communicate assessment findings and needs of admitted patients to members of the receiving unit’s interdisciplinary team
  • Promote a model of best practice in Older Adult Care nursing and a senior- friendly philosophy of care within the hospital

Mandatory Qualifications

  • Bachelor’s Degree in Nursing required
  • Registration with the College of Nurses of Ontario (CNO) in good standing
  • CNA Certification in Gerontological Nursing
  • GPA Certification
  • Current Basic Cardiac Life Support certification (BCLS)
  • Minimum of 3-5 years of related clinical experience providing services to a geriatric population with medical, cognitive, mental health, addictions and behavioural complexities.
  • Skills, knowledge and clinical practice with comprehensive Geriatric Assessments
  • Sound understanding and knowledge of the Substitute Decisions Act and the Health Care Consent Act
  • Excellent problem recognition and problem solving skills
  • Ability to work independently and as a part of a team
  • Demonstrated understanding of and sensitivity to social and cultural diversity
  • Highly developed organizational, communication and problem solving skills
  • Knowledge of principles of adult education and comfort providing education in group settings
  • Proficiency with MS Office, Meditech, Outlook and various database software
