A healthcare institution in Ontario seeks a licensed physician for a clinical position in geriatric consultation. The role includes leading initiatives, providing comprehensive assessments, and collaborating with a multi-disciplinary team. Position Summary
This is a clinical position responsible for the provision of clinical geriatric consultation services. This successful candidate will provide leadership and consultation for Senior Friendly Initiatives within OSMH, collaboration with the North Simcoe Muskoka Specialized Geriatric Services (NSMSGS) to support a Regional Integrated Geriatric System and participation in the Couchiching Ontario Health Team. Participation in the Internal Medicine on-call service is optional. Locum opportunities may be arranged as part of the recruitment process.
Geriatric Consultation :
Provides in-patient comprehensive geriatric assessments for hospitalized patients and patients admitted to the Acute Care of the Elderly Service in collaboration with our multi-disciplinary team members.
Participates in the development of strategic and operational plans for future Senior Friendly care (delirium prevention, ALC days, hip fracture order set review, etc).
Seniors' Care clinic :
Work as a consultant within the well-established outpatient multi-disciplinary Geriatric Outreach Team (GOT). Offer CGAs and longitudinal care for complex older adults with a variety of presenting issues (Dementia, Parkinson’s Polypharmacy, Falls, etc).
The GOT clinic is embedded within the Couchiching Family Health Team and the team includes a Nurse Practitioner, 2 Occupational therapists, 3 Registered Nurses, 1 kinesiologist and administrative support. We work with NSMSGS for centralized intake for all geriatric referrals.
Remuneration : Fee For Service (FFS)
Qualifications
Applicant must be a licensed physician in the Province of Ontario or hold applicable FRCPC designation (or requisite eligibility) or American Board certification, with eligibility to obtain a license to practice in Ontario.
Applicant must be eligible for appointment to the Credentialed Staff at Orillia Soldiers’ Memorial Hospital.
