Overview

The Geriatric Specialist for the Nursing Led Outreach Team (NLOT) demonstrates a high level of autonomy and expertise in geriatric care to formulate clinical decisions. The Geriatric Specialist on the NLOT appropriately supports geriatric patient care for patients from Long Term Care Homes(LTCHs) within the catchment area with acute / chronic illness and promotes wellness and optimization of their current health condition while utilizing their expert assessment skills to ensure patients receive the best care.

The principal responsibilities include utilizing and demonstrating a comprehensive theoretical knowledge base level of clinical competence in caring for elderly patients. The Geriatric Specialist NLOT collaborates and consults with the interprofessional team within the LTCHs and in hospital to ensure comprehensive assessment, analysis, treatment, planning and evaluation of resident\'s care and capacity building with staff to improve the quality and scope of care provided to residents in LTCHs.

Education
  • Registered Nurse with current Certificate of Registration issued by the College of Nurses of Ontario, BSCN preferred.
  • GNC (Geriatric Nurse Certificate) preferred.
  • Current BLS mandatory.
Qualifications
  • Experience in providing effective professional practice leadership in a healthcare setting.
  • 5 years of experience in Gerontology.
  • Advanced health assessment : Experience in Emergency Department setting is preferred.
  • Knowledge of current and future trends in Geriatric Emergency Management (GEM).
  • Knowledge of community programs, clinics and networks for the geriatric population.
  • Excellent verbal and written communication skills.
  • Excellent problem solving, critical thinking and teaching skills.
  • Advocate for patients, staff, and the advancement of professional practice.
  • Visible, approachable and dedicated to follow up.
  • Works effectively as a team member.
  • Strong computer skills utilizing Microsoft Office Suite products.
  • Demonstrated commitment to lifelong learning.
