Qualifications

• Bachelor of Nursing or Master’s Degree in a health-related discipline required.
• Active registration with relevant Ontario regulatory college(s).
• Minimum 3 years of mental health experience.

Responsibilities

• Coordinate and provide clinical care that supports recovery for diverse mental health challenges.
• Triage, perform mental health assessments, develop treatment plans, and navigate resources with the inter-professional team.
• Demonstrate knowledge of senior mental health, addiction, and home-based interventions to facilitate referrals and linkages.
