The Social Worker/Social Service Worker reports to the Executive Director and is responsible for providing orientation, informal education, support, and counseling to residents, families, and friends to promote comfort, well-being, and empowerment. Interventions will be necessary in interactions among individuals, families, staff, and the community.

Responsibilities
  1. Develop, implement, and evaluate social service programs.
  2. Support department managers in staff interventions when needed.
  3. Facilitate support groups for families and residents.
  4. Provide bereavement counseling for residents, families, and staff as needed.
  5. Coordinate Family Council meetings.
  6. Act as the Home Resident Placement Coordinator.
  7. Participate in tours of the Home with potential applicants and their representatives.
  8. Maintain a waitlist in conjunction with Home and Community Care Support Services.
  9. Communicate with placement agencies, applicants, and their representatives to ensure occupancy targets are met.
  10. Maintain outreach services by liaising with hospital social workers, discharge planners, community agencies, and other referral sources.
  11. Understand and participate in quality indicator initiatives and improvement plans.
  12. Participate in recruitment, orientation, retention, and performance management activities.
  13. Review resident satisfaction data monthly and quarterly, with action plans for improvement.
  14. Perform other duties as assigned.
Qualifications

The qualifications needed include:

  • Diploma in social services, gerontology, or a degree in Social Work.
  • Registration under the Social Work and Social Services Work Act with a current certificate (GPA, U-First!, or P.I.E.C.E.S).
  • Or previous experience as a Social Worker or Social Services Worker in a Long-Term Care Home with relevant experience within the last five or seven years.
  • Experience in geriatric social work is an asset.
  • Strong interpersonal, communication, and organizational skills.
  • Knowledge of RAI-MDS; certification or the ability to obtain it within one year.
  • Respect for individual dignity, autonomy, and rights.
  • Second language skills are an asset.
  • Commitment to confidentiality and legal requirements, including a Vulnerable Sector Check and TB testing.
  • COVID-19 vaccination is required.
