Horizon Health Network (Horizon) is looking for a motivated and flexible individual with experience in Addiction & Mental Health to fill the role of Prevention Coordinator.

Who you are

You have worked in Addiction & Mental Health for 3-5 years, with experience in community development and health promotion. You understand the health needs of the populations served and are knowledgeable about suicide prevention strategies and the determinants of health. You have experience working with community groups, can work independently, and are willing and able to travel regionally. Flexibility to work evenings and weekends is required. You possess excellent interpersonal, oral, and written communication skills, and are proficient in Microsoft Office applications. You hold a University Degree in social work, health sciences, or social sciences with 3-5 years of experience, or a college certificate/diploma with 7-10 years of experience. Fluency in English is required; bilingualism is an asset but not mandatory.

What you will do

You will facilitate information sessions and community consultations to identify needs and develop solutions related to addiction and mental health. You will coordinate and design community action plans in partnership with citizens and stakeholders, aiming to meet community needs. Your responsibilities include designing and evaluating community programs, building community support networks, supporting the formation of groups to develop strategies, and implementing solutions. You will work to develop and sustain partnerships to endorse regional suicide prevention frameworks, create linkages among services, and support self-help groups. Managing the suicide prevention budget and exploring funding opportunities with stakeholders to enhance resources are also key duties. Additional responsibilities may be assigned as needed.
