HOUR S: Permanent Full-time (37.5 hours per week), Monday - Friday, 8:30am - 5:00pm

REPORTS TO: Housing First Team Lead

Reporting to the Housing Choice Program Manager, the Follow Up Support Worker manages a caseload of previously homeless individuals and/or families to maintain their housing by supporting them in accessing community resources and services to meet both their basic and complex needs. The role involves working in partnership with landlords and other agencies to ensure coordinated services.

The purpose of this role is to assist individuals in maintaining their housing status once they achieve a degree of independence. The FSW helps clients access services and programs that address their immediate and long-term needs, engaging routinely with clients to ensure their needs are met and fostering skills and habits that improve their quality of life.

Key Areas Of Responsibilities
Program Supports
  • Deliver and maintain the Housing First program in alignment with BSCS core values.
  • Build rapport with participants to develop strength-based, person-centered case plans.
  • Manage a caseload of up to 20 participants.
  • Assess barriers and needs with participants to identify necessary supports and resources.
  • Advocate and support participants for medical, legal, financial appointments, referrals, and transportation.
  • Coordinate referrals to community support services such as employment supports, home care, mental health care, treatment & recovery facilities, and Children & Family Services.
  • Develop crisis plans with participants.
  • Support skill-building in personal hygiene and household maintenance.
  • Maintain monthly contact with landlords and foster positive interactions between participants and landlords or resident managers.
Administration
  • Maintain detailed case notes within 48 hours of interactions.
  • Conduct assessments every 3 months and complete other program-related administrative tasks.
  • Ensure all documents are current and uploaded to Efforts to Outcomes (ETO).
  • Complete financial documentation accurately and on time for leadership approval.
Miscellaneous
  • Provide overdose response, crisis intervention, and de-escalation as needed.
  • Assist with short-term housing supports with BSCS partners.
  • Perform other duties as assigned by the Team Lead and/or Manager.
  • Participate in community events, fundraising, and other organizational activities, including occasional weekend and evening work.
Education, Experience & Skills
  • Degree in Human Services or related field; relevant experience with post-secondary education considered.
  • Strong commitment to BSCS values.
  • Excellent organizational, multi-tasking, and problem-solving skills.
  • Respectful, client-centered approach.
  • Proficiency in Microsoft Office suite.
  • De-escalation skills for handling aggressive clients.
  • Minimum of 2 years experience in community-based services.
  • Knowledge of Landlord and Tenant Legislation.
  • Experience with evidence-based practices, motivational interviewing, and cognitive behavioral techniques.
  • Experience networking with partners and stakeholders.
  • Understanding of poverty-related issues, especially within urban Indigenous populations.
Training & Screening Requirements
  • Valid First Aid certificate.
  • Up-to-date Police Information Check (every 3 years).
  • Child and Youth Intervention record (every 3 years).
  • Valid Class 5 driver’s license and reliable vehicle for local travel.
