Job summary

Canadian Addiction Treatment Centres in Ontario is seeking a Float Patient Care Coordinator to provide general clinic support and assist with patient needs, ensuring confidentiality and effective communication with residents, families, referents, and co-workers.

The role requires scheduling and data management in Accuro, coordination of treatment plans with the healthcare team, and additional office duties. A Medical Office Administrator certificate and strong Microsoft Office skills are assets.

Qualifications

• Completion of a Medical Office Administrator program or similar certificate is an asset.
• Strong commitment to working with individuals living with addiction and mental health.
• Previous experience in Customer Service or with vulnerable populations with addiction and mental health is an asset.
• Provision of a recent and acceptable Criminal Reference Check.
• Current First Aid and CPR Certification is an asset.
• Experience billing through EMR would be an asset.
• High proficiency with Microsoft Office applications (Word, Excel, PowerPoint, Outlook).
• Excellent interpersonal and communication skills; ability to clarify priorities amongst responsibilities.
• Detail oriented, accountable with good organizational and time management skills.

Responsibilities

• Greets all patients and responds to inquiries in-person, by phone, and by email.
• Maintains competency with Accuro for scheduling, data entry, assessment and documents.
• Performs follow-ups to confirm appointments.
• Communicates with patients about appointments and referrals.
• Collaborates with the interdisciplinary team to plan and evaluate treatment plans.
• Liaises with physicians, managers, counsellors and pharmacy staff for patient needs and emergencies.
• Assists with general office administration and supplies.
• Maintains patient data in Excel to keep information accurate.
