About the Position:

The Float Patient Care Coordinator provides general support to the clinic. The ideal candidate is someone who has an understanding of addiction and co-occurring disorders and is comfortable in dealing with people in cases where they may need to help de-escalate and stabilize patients. A professional demeanor and the ability to communicate effectively with residents, family members, referents, and co-workers are required. The ability to maintain confidentiality of information is essential.

YOUR DUTIES AND RESPONSIBILITIES:

  • Greets all patients and responds to all in-person, phone, and email inquiries from patients and families.
  • Maintains a high-level of competency with Accuro, scheduling appointments, information entry and assessment, data management, and document management.
  • Follows-up with patients to confirm appointments
  • Communicates with patients regarding appointments and external referrals if applicable.
  • Collaborates with members of the interdisciplinary team to plan, implement, coordinate, and evaluate the appropriate treatment plans to support the patient.
  • Communicates with physicians, office managers and counsellors and pharmacy team around patient needs, change in status and all emergency situations.
  • Assists with office administration including ordering supplies and other duties that may be assigned.
  • Maintaining patient data information within excel to ensure all patient information is accurate and up to date.

WHAT YOU’LL NEED TO HAVE:

  • Completion of a Medical Office Administrator program and or similar certificate is an asset.
  • Strong commitment to working with individuals living with addiction and mental health.
  • Previous experience in Customer Service or with vulnerable populations with addiction and mental health is considered an asset.
  • Provision of a recent and acceptable Criminal Reference Check.
  • Current First Aid and CPR Certification is an asset.
  • Previous experience billing through EMR would be considered a definite asset
  • High Proficiency with Microsoft Office applications such as Word, Excel, PowerPoint, and Outlook
  • Excellent interpersonal and communication skills, with an ability to clarify priorities amongst responsibilities.
  • Strong work ethic, applies judgment and discretion in his/her work
  • Detail oriented, accountable with good organizational and time management skills
  • Demonstrated ability to learn quickly and excellent problem-solving skills
  • Ability to work well with others in a fast-paced environment.
