Ontario Health atHome is seeking a Float Care Coordinator to assess patient needs and coordinate care services across various settings in the Ontario Health atHome Central East geographical region. This full-time role requires a committed professional with experience in community health and proficiency in building effective relationships with patients and multidisciplinary teams. As a Float Care Coordinator, you will assess and determine patient care needs and eligibility, provide accessand referrals to community services, and engage with patients, caregivers and other health care practitioners.

This position will provide float coverage for all patient services programs. The position may involve travel to all branches and hospital sites throughout the geographical region of Ontario Health atHome Central East.

Hours of work:
All hours of operation, including extended hours (70 hours per bi-weekly pay period).
Position location and travel:
Hybrid work model, subject to change.
Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
