The ideal candidate holds current Ontario nursing registration, with 3–5 years in primary care, BCLS, and strong EMR skills. You will engage in patient education, triage, and collaborative teamwork in a fast-paced setting.

Qualifications

• Current registration in good standing with the Ontario College of Nurses.
• Bachelor's degree in Nursing preferred.
• Basic Cardiac Life Support certificate required.
• 3–5 years in a Primary Care/Family Practice setting with relevant clinical areas.
• Strong EMR experience and computer skills.
• Ability to make autonomous nursing decisions and triage.
• Demonstrated teamwork, communication and teaching abilities.
• Excellent time management and organizational skills.
• Professional conduct and safety advocacy in care delivery.

Responsibilities

• Provide direct nursing care and assist in teaching students.
• Coordinate daily team activities for patient care delivery.
• Accurately record patient health information.
• Arrange follow-ups and referrals to community services.
• Assist patients in accessing appropriate health care.
• Review test results and communicate with patients per standards.
• Triage patient calls and provide appropriate advice.
• Guide patients on goals and refer to community resources as needed.
• Collaborate with physicians and staff to ensure efficient care.
• Perform procedures such as immunizations, injections, dressings, ear care.
