Job summary

HealthOne is expanding its Family Medicine team and hiring Family Physicians for downtown Toronto clinics. This is a long-term practice opportunity with strong clinic support within our FHO+ model to deliver longitudinal, patient-centered care.

You will work closely with a multidisciplinary team to provide comprehensive care, focusing on continuity and patient outcomes, across Metro Center, The Well, and Harbourfront locations.

Qualifications

• Licensed to practice as a Family Physician in Ontario (CPSO in good standing).
• Willing to work 4 days per week plus one weekend day per month.
• New grads welcome; 1+ year experience preferred.

Responsibilities

• Build strong relationships with patients and deliver exceptional care.
• Support acute, preventive, and chronic disease needs with clear education and continuity.
• Apply full scope with a multidisciplinary team to plan diagnoses and treatments.
• Engage with the team and stakeholders through on-site education and care initiatives.

About the Role

We’re growing our Family Medicine team and recruiting Family Physicians to join our downtown clinics. This is an opportunity to build a long-term family practice with strong clinic support and to play an important role in delivering longitudinal, patient-centered care within our FHO+ model.

As part of our FHO+ model , you will work closely with a multidisciplinary team to provide comprehensive care and support continuity for your patient panel. Our clinics offer a collaborative environment with the infrastructure and resources needed to support physicians in building and growing their practice .

Full-time Family Physician - FHO+ Locations: Metro Center, The Well and Harbourfront

Responsibilities

• Build strong relationships with patients and provide an exceptional care experience, with a focus on compassion and clinical excellence
• Support your patients with their acute care, preventative care, and chronic disease management needs with clear education, shared decision making, and appropriate continuity
• Apply your full scope to create thorough diagnoses and treatment plans which utilize our multidisciplinary team to support patients with all interconnected aspects of their health journey
• Engage with our multidisciplinary team and internal stakeholders through on-site education sessions and care innovation initiatives across HealthOne

Qualifications and Skills

• Patient focused and service minded approach to delivering excellent, comprehensive care
• Empathetic, collaborative approach and ability to work effectively with a wide range of internal and external stakeholders
• Commit to working at HealthOne for a minimum 4 days per week + 1 weekend day per month, with reliable scheduling
• Licensed to practice as a Family Physician in Ontario and in good standing with the CPSO
• 1+ year of experience preferred, however new grads are also welcomed
