Bear River Medical Centre in Bear River, NS is seeking a physician to work one day per week or two days bi-weekly. Role includes delivering comprehensive primary care, collaborating with local providers, and participating in potential inpatient rotations at nearby hospitals.

Qualifications

Responsibilities

• Deliver comprehensive primary health care to patients of all ages.
• Be accountable for workload and practice management.
• Coordinate continuous patient care with other providers and community.
• Provide care across community-based, secondary and tertiary services as needed.
• Develop capacity for new primary care providers within two years.
• Operate within NS Health Authority bylaws and regulations.
• Strive for same- or next-day access for patients.
• Collaborate with team members to meet community health needs.
• Ensure after-hours access and emergency coverage when required.
• Engage in quality improvement and EMR documentation for patient records.

Job description

The Bear River and Area Community Health Clinicis seeking a physician to work one day a week or two days bi-weekly. This clinic services patients who live from Annapolis to Weymouth. The physician may wish to participate in the inpatient rotation at Digby General Hospital or explore other options in the Annapolis to Weymouth communities if collaboration with other providers is of interest.

Responsibilities

• ensure the delivery of comprehensive primary health care services to patients of all ages and stages of health or illness
• be accountable for appropriate workload and practice
• continuum of patient care with health care providers within your practice and/or community
• provide care and coordination of community-based care, secondary and tertiary level heath care
• build to capacity within the first two years (new primary care providers)
• operate within the Bylaws and Rules and Regulations of Nova Scotia Health Authority
• work toward same day or next day access for the practice population
• work in collaboration with other team members in the practice to best suit the community’s health needs
• work with other providers to provide after-hours access and emergency coverage when required
• participate in quality improvement and evaluation activities
• utilize an electronic medical record (EMR) and record all relevant patient details in a provincially approved EMR which is accessible to all collaborating team members

• part-time - one day/week or two days/bi-weekly

Remuneration

• Remuneration type is eitherLongitudinal Family Medicine payment modelor a Fee for Service paymentmodel.
