Job description

GENERAL DESCRIPTION OF ROLE AND DUTIES

The role of physician is to provide high quality primary health care patient services to support the Mission and Vision of the Health Center. The physician will support the organizational values and strategic goals including care delivery thru our patient centered medical home model. The physician will document in the electronic health record per protocol. The Physician will provide services in compliance with applicable regulatory and licensing agencies.

The physician is a member of the clinical team. The physician, unless otherwise noted, does not have formal supervisory authority over other Center staff. However, the physician is expected to monitor and provide counsel, collaboration, and consultation to other clinical and administrative staff to achieve the best possible clinical outcomes for Health Center patients. The Physician will actively participate in staff meetings, trainings, and performance reviews. The Physician will support the Quality Improvement plan and goals including initiatives to support patient safety, the patient experience, access to care, and initiatives to improve the health of the community. The Physician will actively participate in the Peer Review process. The Physician will report to the Chief Medical Officer.

This job description applies to any physician practicing in any location associated with the Health Center and approved within the scope of project by HRSA.

ESSENTIAL FUNCTIONS

  • Provides comprehensive primary care within the scope of practice defined by the Health Center privileging protocol for the medical specialty. Care will be delivered in full accordance with HCHC policies and protocols, including:
    • Provide wellness, chronic disease, same day, and emergency services in accordance with the patient’s needs.
    • Provide a clear assessment and safe treatment plan for each patient encounter that considers the patient’s circumstances.
    • Order, perform, and interpret routine laboratory and diagnostic imaging tests and notify patients of results per HCHC protocol.
    • Provide access to primary care thru Telehealth and phone.
    • Support wellness and restoration through self-care, continuity of care and care coordination.
    • Refer patients for specialized care based upon an identified need. Incorporate recommendations from consultants into treatment plans.
    • Complete documentation in the EMR per protocols. Comply with EMR maintenance and upgrade activities.
  • Supports and provides collaboration, supervision and consultation to physician assistant(s)/nurse practitioner(s) and clinical team in to plan and coordinate patient care.
  • Performs all job activities in ways that are respectful, courteous, and responsive.
  • Interacts with coworkers in a respectful, supportive, and cooperative way
  • Provides professional expertise for in-service training and Quality Improvement projects.
  • Actively participates in Quality Improvement initiatives and Peer Review
  • Obtains and maintains appropriate privileges at area community hospitals.
  • Attends internal and external meetings, workshops, conferences, and participates in task forces or committees as necessary and/or required.
  • Demonstrates commitment to our recognition as a Patient Centered Medical Home and its associated excellence standards by modelling excellence in all clinical, administrative and team process relationships, addressing service excellence deficits.
  • Provides call coverage, cross coverage, Saturday clinic coverage and other shared work.
  • Other duties and responsibilities as assigned by the Chief Medical Officer or the Executive Director.

QUALIFICATIONS AND REQUIEMENTS FOR POSITION

Maintains federal, state, and professional licenses/qualifications to maintain scope of practice in Virginia. Unrestricted license to practice medicine in the Commonwealth of Virginia. DEA license to prescribe medicine. Board certification in chosen field of practice. Cannot be sanctioned under Medicaid or Medicare. Current CPR (BLS) certification is required.
