Job summary

Lumenus Community Services in Toronto seeks a Family Home Visitor to support families with young children (birth to six) who are at risk, providing parenting guidance and connecting families with community resources.

This one-year, full-time fixed-term position involves home visits, goal setting, documentation, and collaboration with public health nurses and agency partners to promote healthy child development. Fluency in Urdu, Hindi and Punjabi is required to serve diverse client populations.

Qualifications

• College Diploma in Early Childhood Education with registration in good standing with the College of Early Childhood Educators.
• 3-5 years' experience working with children and families in community settings.
• Experience communicating with people from diverse ethno-cultural backgrounds.
• Familiarity with the challenges of families who are new to Canada.
• Fluent written and verbal communication in Urdu, Hindi and Punjabi.

Responsibilities

• Provide parenting support and community resource information to families with young children who are at risk on the basis of social isolation.
• Plan home visits to meet goals/objectives.
• Assist families to identify and build on the strengths of their children.
• Administer the Nipissing District Developmental Screens (N.D.D.S.).
• Model positive parent-child interaction.
• Provide parenting information specific to healthy growth and development e.g. breastfeeding, nutrition, stimulating play activities, home safety etc.
• Assist families to link with parenting programs and other community resources as needed.
• Document all family visits and maintain all records and forms required for family files in a timely fashion.
• Report to Manager any significant family concerns.
• Maintain ongoing communication with community partners (Public Health Nurses, Children's Aid Society Workers, etc.).
• Participation in monthly Team Meetings.

SALARY RANGE: $49,600 - $62,000

The Family Home Visitor is responsible for providing parenting support and community resource information to families with young children (birth to age six) who are "at risk" on the basis of social isolation (i.e. new to the country and away from family and friends), economic insecurity, language barriers, etc.

The Family Home Visitor works in partnership with Toronto Public Health Nurses and other community agency staff members to ensure that families are supported in assisting their children to grow to their optimal potential.

KEY RESPONSIBILITIES:

Provide parenting support and community resource information to families with young children (birth to age six) who are "at risk" on the basis of social isolation

QUALIFICATIONS/EXPERIENCE:

• College Diploma in Early Childhood Education and, registration in good standing with the College of Early Childhood Educators is a must, or a High School Diploma coupled with relevant experience
• 3 -5 years' experience working with children and their families in community settings
• Experience in communicating with people from diverse ethno-cultural backgrounds
• Familiarity with the challenges of families who are new to Canada
• Personal parenting experience is an asset
• Fluent written and verbal communication in Urdu, Hindi and Punjabi is a must in order to properly service the client population
