Job summary

Manitoba Possible is seeking a Children’s Service Coordinator to provide holistic, family-centered service coordination for children with disabilities and their families. You will manage a dynamic caseload, develop and monitor individualized service plans, and collaborate with the family, program staff, and collateral systems to address evolving needs.

Qualifications

  • Bachelor of Social Work with 3-5 years of service coordination/case management experience.
  • Knowledge of human behaviour, growth, and child development.
  • Knowledge of neurological and/or physical disabilities and their impacts on children and families.
  • Knowledge of Inclusion Philosophy and family-centered service delivery.
  • Strong communication skills and ability to work in a team.
  • Basic knowledge of computers – Microsoft Office; driver’s license required.

Responsibilities

  • Develops, implements, monitors and evaluates comprehensive service plans in partnership with families.
  • Provides leadership in integrated service management aligned with agency mission and values.
  • Establishes collaborative relationships with families and significant others to maximize client participation.
  • Reviews and revises service plans through meetings to ensure goal setting and completion.
  • Provides information and practical help to clients and promote self-leadership.
  • Monitors progress and ensures timely service delivery and proactive planning.
  • Participates in supervision, team meetings, and relevant training.
  • Maintains the Manitoba Possible case management system and community resource links.
