Job Summary

The HCA / Caregiver is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the clients needs.

Responsibilities

  • Assistance with personal care and hygiene
  • Meal Preparation
  • Accompany clients on outings
  • Shopping and errands
  • Medication Assist / Reminders
  • Ambulation and transporting of clients
  • Provide companionship to clients
  • Ability to follow care-plan
  • Documentation / Charting

Qualifications
