Exceptional Family Member Program (EFMP) Coordinator

Minimum Qualifications:

  • Degree: Master of Social Work (M.S.W.) from an accredited school by the Council on Social Work Education (CSWE).
  • Certification: Current, unrestricted clinical social work license in any U.S. state, D.C., Puerto Rico, or U.S. Virgin Islands.
  • Experience: At least two years of full-time experience in care coordination, discharge planning, or case management in a medical setting within the last four years.

Required Skills/Abilities:

  • Oversee and manage the installation EFMP-M according to DoD and military policies.
  • Use clinical assessment skills to identify sponsors with family members who have special needs, ensuring timely intervention and collaboration with service providers.
  • Manage data entry and oversight in military data management systems related to EFMP-M.
  • Coordinate the enrollment process, travel screening, and relocation clearance for families.
  • Maintain and update files for sponsors with family members with special needs, and ensure annual contact for needs assessment.
  • Collaborate with interagency forums and community resources to support EFMP families.
  • Participate in case reviews, meetings, and training related to EFMP services.
  • Complete medical record documentation, coding, and reporting as per policies.
  • Maintain knowledge of EFMP assessment, planning, facilitation, and advocacy functions.
  • Demonstrate psychosocial practice skills, understanding of human development, and program evaluation capabilities.
  • Work effectively with service members and veterans experiencing complex issues.
  • Ensure compliance with HIPAA, accreditation standards, and confidentiality protocols.
  • Proficient in Microsoft Office, data management, and communication skills.
