A leading home health care provider is seeking Enhanced Palliative Personal Support Workers in Kitchener to deliver 1:1 care for patients. Responsibilities include providing palliative and complex care, working with nurses through a virtual care platform. Requires a valid PSW certificate or equivalent. New grads may receive bonuses up to $20,000 and relocation bonuses. Full-time with 12-hour overnight shifts including weekends.

Overview

As a progressive and creative organization, CarePartners is proud to be a leader in developing innovative programs. One of our spotlight programs is the eHomeCare Program. This is an innovative provincial program that is unique to home care and includes oversight of the patient through a virtual care platform.

We are looking for energetic and dedicated Personal Support Workers to help patients in our community, with activities of daily living. You will help provide palliative and complex care, and stable pediatric care for children with complex needs. You will provide care to patients in their own homes, working collaboratively with specially trained nurses who will assist you through supportive technology. CarePartners will provide education specific to eHomeCare, as well as a comprehensive, fully paid orientation.

Hours of work will be overnights 8 pm - 8 am Monday-Friday including a rotation of weekends.

What The Role Involves
  • Delivering care and support to patients and their families in their home
  • Providing quality, integrated care through direct communication between a nurse via laptop/tablet and the eHomeCare Support Worker via smartphone throughout their shift
What You Bring
  • A strong desire and commitment to making a difference in the lives of their patients
  • Exceptional interpersonal skills
  • Ability to work both independently and as part of a team
  • Availability to work non-traditional hours, such as early morning, evenings, and alternate weekends
  • A valid driver’s license and vehicle
  • Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aide, Developmental Support Worker, or completed 1st year in a Nursing program
  • To protect our patients, we require current vulnerable sector check screening, a clear background check

New Ontario PSW grads may be eligible to receive a $10,000 bonus

from an approved PSW program in Ontario (on or after April 1, 2024)

and an additional $10,000 relocation bonus

when relocating to rural, remote or northern area and committing to working with us for 12 months, payable at start and 6 months
