Qualifications

  • Must be actively registered with the BCCNM as an RN.
  • Wound care experience is required.
  • Excellent written and verbal English skills; other languages are an asset.

Responsibilities

  • Provide treatment services that are inclusive and flexible for individuals with mental illness, addictions and/or chronic conditions.
  • Collaborate with community health clinics, hospitals, housing providers and interdisciplinary teams for appropriate care.
  • Perform intake, health assessments, planning, crisis intervention, and care coordination.
  • Supervise and support staff to mitigate risks and ensure safety and wellness.
  • Direct health care aides; assign tasks and ensure policies are followed.
  • Engage program participants in decision-making to improve identity and self-esteem.
  • Maintain accurate case notes and files per accreditation standards.
  • Ensure compliance with Society policies and reporting obligations.
  • Other duties as required.

Job description

Job Title: Enhanced Care Nursing Coordinator RN
Work Location: 317 Tranquille Rd.
Reports To: Chief Operations Officer

Annual Salary: $47.26-57.84

Schedule: Up to 18.50 hours per week/flexible work schedule

Position Summary: Assess, identity, plan, implement and evaluate the care required to assist enhanced care clients in meeting their physical, psychiatric, social and spiritual needs. The Nursing Coordinator works within the team’s interdisciplinary clinical framework to provide client centered and strengths-based care in accordance with the BCCNM Standards of Practice and consistent with the mission, vision and values and established policy and protocols of the Society.

Specific Responsibilities:
  • The Nursing Coordinator provides treatment services that are both inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions and ensures integration and continuity across the care continuum;
  • The Nursing Coordinator establishes and maintains relationships with, and consults with community health clinics, hospitals, housing providers and the interdisciplinary teams to provide appropriate treatment services based on best practices and client need;
  • The Nursing Coordinator performs duties such as, intake, mental and physical health and substance use assessment, treatment planning, crisis intervention, consultation with hospitals around care planning and discharge, wound care and medication monitoring and administration;
  • Provide supervision and support to mitigate individual and program risk to ensure safety, comfort, and wellness;
  • Directs care aides (health care assistants) by assigning daily tasks, supervising client care, and ensuring policies and procedures are met;
  • Actively support team members to seek the views of Program Participants and involve them in decision-making to improve their own identity, purpose, and self-esteem;
  • Complete accurate and up-to-date communication/case notes;
  • Maintain files and statistics as per accreditation (COA) standards;
  • Ensure compliance with all Society policies and procedures;
  • Ensure contractual and reporting obligations are met;

Qualifications and Requirements:
  • Actively registered with the BCCNM as an RN.
  • Wound care experience is required.
  • Excellent written and verbal English skills. Other languages an asset.
  • Class 5 Drivers License & reliable transportation
  • Ability to provide training, orientation, consultation, and guidance within clinical specialization of practice.
  • Strong ability to recognize and address difficult behaviours.
  • Good communication, documentation, interpersonal, and conflict management skills.
  • Ability to work within a team-based environment involving multi-agency support.
  • Strong working knowledge of community resources.
  • Must pass an acceptable criminal record check for the vulnerable sector.
  • Current Standard First Aid with CPR.
  • Mental Health First Aid, ASIST, and Trauma Informed Practice an asset.
  • As Program Participants are allowed to have pets, it is noted that this is a pet-friendly environment.

  • Effective verbal and written communication skills necessary for working with persons with varied social behaviours
  • Ability to work within an multi disciplined team environment
  • Ability to observe and recognize changes in clients
  • Ability to establish and maintain rapport with clients
  • Ability to organize and prioritize
