Job Overview:

The Community Health Worker will primarily maintaining a presence in the community with patients to promote, motivate and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Qualifications:

Required:

  • High School Diploma or equivalent

Preferred:

  • Bi-lingual in Spanish is strongly preferred

Scheduling Requirements:

  • Full-Time, Evening
  • Working Location: NBR- New Brunswick, NJ

Essential Functions:

  • Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using Social Determinants of Health Tool.
  • Schedule weekly follow-up calls and home visits to patients.
  • Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives.
  • Assist patients with organizing their records, making follow-up appointments with a Primary Care Physicians or Specialists.
  • Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise.
  • Help patient s access resources, including quality of care, childcare services, food, housing, transportation.
  • Maintain relationship with the Community Based Organization.
  • Able to travel and meet the members where they at; may include local Churches, homeless shelter, and members residence.
