Position OverviewThe Home Care Program provides a broad range of services to assist people to safely remain in a community setting. Under the general supervision of the Manager Health Services and while demonstrating a commitment to the mission, vision and values of the WRHA, the Hospital Based Case Coordinator (HBCC) receives referrals and conducts assessments to determine eligibility for Home Care program supports. The HBCC in collaboration with the client, family/carer and relevant interdisciplinary team members, develops, coordinates and evaluates the plan of care to support discharge from an acute care setting. In addition, through the collaborative process, the HBCC assists in the determination and planning if the client’s needs are best supported in a personal care home or alternate care environment. The HBCC provides professional intervention where appropriate. Additionally, the HBCC establishes and maintains liaison with local health care services, hospital, community partners, and their families/carers involved with the client.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
