Job description

Title: Transition Home Support Student

Employee Working Location: In person - Timmins ON

Employment Status: Temporary Full-time (2 Months - Summer Student)

In this role, you will:
  1. Provide support to seniors during hospital discharge.
  2. Support clients during the transition from hospital to home in accordance with the established service plan.
  3. Complete basic errands for service participants as required.
  4. Conduct basic home management duties.
  5. Complete risk analysis of home using standardized Risk Assessment tools.
  6. Facilitate engagement of service participants and caregivers.
  7. Facilitate referrals to Community Support Services.
  8. Handle and store hazardous products safely.
  9. Maintain the comfort and safety of the service participants' immediate environment.
  10. Identify and report any unsafe or hazardous equipment or situations to the Program Manager.
  11. Take measures to ensure personal safety and to protect the health and safety of service participants.
  12. Observe and report any concerns related to the service participant and/or their environment to the Program Manager.
  13. Provide companionship to service participants during transition home.
  14. Assist the Program Manager with statistics and reports as needed.
What we are looking for:
  1. Excellent interpersonal skills and the ability to accurately document and record information.
  2. Strong computer skills, particularly in Microsoft Office applications including Word, Excel, and Outlook.
  3. Proven organizational abilities with a keen eye for detail and the capacity to manage and interpret complex information.
  4. Outstanding written and verbal communication skills.
  5. A proactive mindset with the ability to take initiative and work independently.
  6. Solid problem-solving skills and the ability to troubleshoot and resolve issues effectively.
Working conditions:
  1. Be between 16 and 30 years of age at the start of employment.
  2. Have a valid Social Insurance Number.
  3. Valid Driver's license.
  4. Applicants whose first language is not English may be required to perform responsibilities in English, as we work with and support diverse communities.
  5. Eligibility to work in Canada: Applicants must be eligible to work in Canada; those not citizens or permanent residents are encouraged to review their visa status.
  6. If selected, you will need to complete a pre-employment screening, including an Enhanced Police Information Check (E-PIC) and driver's abstract.
