Position Overview

Reporting to the Resource Coordinator, the ISW (Integrated Support Worker) will have knowledge and training in personal care skills and be a certified Health Care Aide (HCA). Preferably, the ISW will also have additional training in working with individuals experiencing mental health or behavioral challenges, with training provided after start date. The ISW is responsible for client care involving HCA duties and supportive care duties as directed by the Case Coordinator/Resource Coordinator. This includes partnering with clients to improve their wellness in community, social functioning, self-care, communication, self-help, and coping. ISWs are located in several housing projects operating in a 24/7 team model, as well as in the community at large.

Experience
  • Two (2) years’ experience as an HCA (Home Care Attendant / Health Care Aide) preferred.
  • Two (2) years’ experience working in Mental Health preferred (may include experience as an HCA working with clients with complex needs or behaviors).
Education (Degree/Diploma/Certificate)
  • WRHA recognized HCA certificate required.
  • Grade XII minimum or GED equivalent.
Certification/Licensure/Registration
  • Valid driver’s license and vehicle required.
  • May be required to provide a Personal Driver’s Abstract.
Qualifications and Skills
  • Knowledge of:
    • Information and/or training will be provided within the probation period for the following:
      • Personal Health Information Act (PHIA), Workplace Hazardous Material Information System (WHMIS), and Principles of Routine Practices (Universal Precautions).
      • Regional and Program policies and procedures.
      • Mental Health First Aid.
      • SafeTALK.
      • WRHA Violent Prevention and Protection Modules and/or Non Violent Crisis Intervention.
      • Roles and responsibilities of members of the healthcare team.
  • General skills and ability to perform all tasks and responsibilities relevant to the duties of the ISW position.
  • Ability to provide safe, effective personal care respecting individual needs for comfort, privacy, and respect.
  • Understanding of client empowerment and issues faced by individuals with mental and/or physical health challenges.
  • Effective communication with clients, family, and coworkers.
  • Ability to assess and teach basic life skills.
  • Flexibility to meet client care needs.
  • Ability to set boundaries with clients.
  • Teamwork and seeking assistance when needed.
  • Effective English verbal and written communication skills.
  • Preference for applicants competent in an Aboriginal language and/or knowledge of Aboriginal customs, traditions, and values.
Physical Requirements
  • Extensive physical activity including lifting, transferring, and transporting patients and equipment.
  • Exposure to infectious diseases, blood, body fluids, toxic materials, noise, allergens, and potential emotional stress.
  • Encountering aggressive or agitated clients/visitors/staff.
  • Travel between clients with unpredictable driving conditions or inclement weather.
  • Home environments with congested spaces, uneven surfaces inside and outside the home.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check, and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is current if obtained within six (6) months prior to employment.

Please note that an employee cannot hold two or more positions across the WRHA legal entity that total more than 1.0 EFT. The WRHA legal entity includes multiple sites and programs.
