Reporting to the Resource Coordinator, the ISW (Integrated Support Worker) will have knowledge and training in personal care skills and be a certified Health Care Aide (HCA). Preferably, the ISW will also have additional training in working with individuals experiencing mental health or behavioral challenges, with training provided after start date. The ISW is responsible for client care involving HCA duties and supportive care duties as directed by the Case Coordinator/Resource Coordinator. This includes partnering with clients to improve their wellness in community, social functioning, self-care, communication, self-help, and coping. ISWs are located in several housing projects operating in a 24/7 team model, as well as in the community at large.
This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check, and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is current if obtained within six (6) months prior to employment.
Please note that an employee cannot hold two or more positions across the WRHA legal entity that total more than 1.0 EFT. The WRHA legal entity includes multiple sites and programs.
