  2. LIC 501 - Personnel Record
  3. LIC 503 - Health Screening (includes TB test)
  4. LIC 9052 - Employee Rights
  5. LIC 9108 – Suspected Child Abuse Statement
  6. Immunization Records (MMR/Tdap/Flu)
  7. Knowledge of Title 22
Additional Requirements

Upon acceptance of the District’s offer of employment, new hires may be required to complete a baseline physical examination prior to the first day of employment. Individuals with transcripts from institutions outside of the United States must obtain a complete evaluation from an approved CCTC agency.
