Provides nursing care to patients under the supervision of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to monitor nursing interventions and assists medical staff to ensure optimal care within established policies, procedures, and standards. Performs nursing duties within the legal scope of practice, demonstrating commitment to the organization's Mission, Vision, and Strategic Plan. Maintains quality work habits, clinical effectiveness, and treats colleagues, patients, families, and all healthcare team members with dignity and respect.
Key Responsibilities:
- Exhibit excellent customer relations with patients, visitors, providers, and co-workers, demonstrating courtesy, compassion, and respect.
- Provide therapeutic patient care as per physician/advanced practice provider or registered nurse directives within scope of practice.
- Transcribe physician/advanced practice provider orders following policy.
- Communicate with registered nurses to modify care plans and contribute to developing nursing care plans.
- Administer medications accurately following provider orders and standing orders within scope of practice.
- Execute treatments such as urinary catheterizations, IV placements, irrigations, suctioning, throat irrigations, enemas, and use of equipment, adhering to infection control policies.
- Maintain high standards of disinfection and sterilization for equipment and supplies.
- Perform patient care functions including vital signs, wound care, patient education, specimen collection, and ensuring proper culture collection.
- Assist medical staff during examinations and treatments, providing support and restraint as needed.
- Observe, document, and report patient conditions, reactions, and unexpected findings promptly, initiating emergency procedures when necessary.
- Volunteer to start and maintain IV therapy and perform venipuncture with appropriate certifications, notifying the team of any issues.
- Collect and process specimens according to policies and protocols.
- Provide non-complicated patient education based on care plans, reporting any learning needs or discharge barriers.
- Ensure cleanliness and proper functioning of nursing stations, patient rooms, and equipment to maintain a safe environment.
- Communicate effectively using verbal and non-verbal methods.
- Perform departmental workflows, including patient outreach, care gap closure, and nurse visits.
- Participate in staff meetings and training programs.
- Assist with ordering, stocking, and distributing supplies and equipment.
- Manage verbal and telephone orders from providers.
- Support patient communication, including managing emails, messages, and call center communications.
- Administer therapies such as pulse oximetry, oxygen therapy, suctioning, and sputum induction.
- Document all care activities in the health record system per policies.
- Assist with discharge processes and orient new employees.
- Perform all duties within the scope of certification and licensure.
- Promote excellent customer service and ensure compliance with organizational principles and legal requirements, including privacy, confidentiality, and ethical standards.
- Utilize skills assessments to ensure proficiency with age-specific patient needs.

- Minimum of 12 months of LVN direct patient care experience within the last three years, including medical office, acute care, home care, or skilled nursing facilities.
