Reporting to the Pharmacy Manager, Clinical Support Services, and under the direction of the Pharmacy Professional Practice Leader, the Pharmacist will provide consultative and professional pharmaceutical services. The Pharmacist will assist in coordinating the activities of the Pharmacy department, oversee drug distribution activities, and ensure drugs are procured, stored, prepared, prescribed, dispensed, and administered safely and effectively. Additionally, the Pharmacist will provide clinical pharmacy services, including comprehensive drug information to support patient care at Providence Care Centre, and promote appropriate pharmacotherapy to optimize patient outcomes. The role involves participating in educational activities related to pharmacy practice. Working collaboratively with clinical, nursing, and allied health staff, the Pharmacist will deliver pharmaceutical care across the continuum of care.
Skills and Qualifications Required
- Bachelor of Science in Pharmacy or PharmD
- Current, unrestricted registration with the Ontario College of Pharmacists
- Successful completion of an accredited Pharmacy Residency Program (preferred)
- Minimum three years of hospital pharmacy experience (preferred)
Position Responsibilities
- Perform therapeutic reviews of medication orders and prescriptions for accuracy and appropriateness
- Identify and communicate drug-related problems to pharmacists or physicians
- Perform final checks on medication orders and prescriptions
- Supervise Pharmacy Assistants and Technicians in drug distribution activities
- Provide direct client care as a clinical pharmacist
- Identify, resolve, and prevent drug-related problems through a collaborative approach
- Individualize medication regimens considering pharmacodynamics and pharmacokinetics; interpret serum drug levels and provide recommendations
- Conduct medication histories, assess adherence, and suggest therapy modifications
- Participate in interprofessional rounds and conferences for patient care planning
- Coordinate with community pharmacists and care providers regarding discharge medications
