Life-Changing Opportunity: Indigenous Patient Navigator at Horizon Health Network

Horizon Health Network in beautiful New Brunswick is seeking an Indigenous Patient Navigator to join our team. This role involves acting as a resource and point of contact for Indigenous clients, their families, healthcare providers, and community members to ensure culturally safe, client-centered care. The navigator will facilitate access to services, assist with discharge planning, and foster an environment that supports traditional Indigenous healing practices using a Two-Eyed seeing approach. The goal is to improve access to appropriate care, increase client capacity, and strengthen connections to support stability and well-being.

Who You Are
  • 1-3 years of recent experience in Indigenous health or social programming
  • Experience advocating for clients and working in fast-paced, evolving settings
  • Proficiency in Microsoft Office applications
  • Strong mediation, conflict resolution, and interpersonal skills
  • Knowledge of Indigenous healing practices, person-centered care, and ability to facilitate smudging ceremonies
  • Understanding of Indigenous health programming at various levels and knowledge of local Indigenous demographics, languages, cultures, and traditions
  • Ability to speak Wolastoqey or Mi’kmaq is an asset
  • Understanding of cultural safety, Two-Eyed seeing, and systemic issues affecting Indigenous health
  • Must be of Indigenous descent
  • Ability to work independently, make decisions, and travel as needed
  • Valid driver’s license and vehicle
Education and Experience
  • Undergraduate degree in health or social sciences preferred
  • Additional certifications in trauma-informed care, birthing or death doula, or willingness to obtain them
  • Experience with Indigenous programming or community connections is considered an asset
Key Responsibilities
  • Support a Two-Eyed Seeing approach to Indigenous client care
  • Engage with Indigenous clients and families to enhance hospital experiences
  • Provide advocacy, health literacy, and empower clients in health decisions
  • Facilitate access to cultural, spiritual, and linguistic resources
  • Support hospital staff awareness of Indigenous resources through education and mentoring
  • Participate in case conferences and discharge planning
  • Document accurately in patient records
  • Collaborate with community stakeholders for seamless care transitions
  • Maintain awareness of Indigenous community resources and develop informational materials
  • Conduct education sessions on Indigenous health for staff and partners
  • Promote understanding of cultural safety and anti-racism initiatives
  • Assist patients with the complaints process and complete workload reports
