Role: Primary Nurse Clinician & Supervisor, Integrated Programs

This is a key clinical leadership position responsible for guiding a team of Registered Nurses (RNs), Registered Practical Nurses (RPNs), and Unregulated Care Providers (UCPs) to deliver advanced, best-practice care in home and community settings. The role combines clinical excellence, leadership, and mentorship to support sustainable care transitions, hospital avoidance, and optimized patient outcomes.

The Primary Nurse Clinician & Supervisor will leverage advanced nursing skills to deliver time-sensitive, comprehensive patient visits—often within 24 hours—to stabilize complex patients, conduct assessments, and develop or adjust care plans. They will identify additional service needs and coordinate with the broader care team for seamless, high-quality care delivery.

In addition to direct care, this role involves training and upskilling team members to deliver more acute and advanced care in the community. They will drive clinical excellence through documentation, care plan development, and sharing critical information across the team, ensuring safety, continuity, and adherence to best practices.

This position emphasizes patient-centered care, team collaboration, and continuous quality improvement to meet the evolving needs of complex patients and transform home and community care delivery.

  • Strong leadership and team development skills
