The Licensed Practical Nurse (LPN), reporting to the Director of Care, is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.
To work as a member of the inter-professional team, applying the person-centered approach to care as described by our Professional Practice Model.
To complete and ensure that residents' clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and that changes are communicated as required.
To implement and evaluate the effectiveness of care/service plans and make necessary adjustments.
To assist with scheduling resident care conferences and participate in them when applicable.
To work closely with the Director of Care to facilitate residents' move-in, move-out, and transfers.
To provide direct resident care, medication/treatment administration, and document care in compliance with regulatory and Resident Care standards, policies, and procedures.
Current registration as a Licensed Practical Nurse in the applicable Province/State.
Experience working in a senior's environment and with unregulated care providers is preferred.
Education and experience in gerontology, rehabilitation, community/complex/long-term care are assets.
Strong written and oral communication skills, critical thinking, care coordination, time management, and customer focus are required.
Demonstrated sound clinical knowledge.
Current Basic Cardiac Life Support and First Aid certifications are required.
Proficiency with computers and experience with Microsoft Office applications.
Ability to provide a complete and current (within six months) Vulnerable Sector Check, including a Criminal Background Check, or willingness to obtain one.
Wage Leveling Salary: $32.84 - $41.35 per hour. Salary placement based on qualifications and experience.
