The Opportunity
The Pharmacist plays a vital role in monitoring and evaluating medication use across the hospital while providing clinical pharmacy support and expertise to physicians, nurses, patients, and other healthcare professionals. This is a position within a collaborative and skilled interprofessional team, with direct patient care opportunities and a focus on quality, safety, and innovation.
Duties And Responsibilities
- Complete patient medication assessments
- Ensure optimal medication therapy and minimization of potential adverse events
- Review of lab information to ensure appropriate drug therapy, oversee therapeutic drug monitoring of appropriate medications
- Participate in clinical rounds
- Participate in drug distribution through verification of physician orders (appropriateness of dose, frequency, route, potential interactions, allergies, contraindications, etc.)
- Assess appropriateness of non-formulary medications and obtain supply or recommend substitution
- Provide healthcare providers with relevant drug information
- Actively review and provide recommendations on various document submissions relating to clinical care
- Work collaboratively with interdisciplinary team on units
- Complete appropriate medication safety audits, such as VTE, opioid waste/discrepancies
- Participate in antimicrobial stewardship activities
- Participate in the education and mentoring of new staff as well as precepting pharmacy students
- Participate in the education and ongoing learning of nursing staff
- Participate in continuous quality improvement of the pharmacy department
- Other duties, as assigned

Education And Experience
- Registered Pharmacist, in good standing with the Ontario College of Pharmacists (Part A)
- Bachelor of Science in Pharmacy or PharmD
- Post-graduate Pharmacy Residency (ACPR), an asset
- Experience in an acute care hospital setting, preferred
- Experience with MEDITECH Expanse and BD Pyxis, an asset
Competencies
- Knowledge of current evidence-based, best practice pharmacotherapeutics & pharmacokinetics and their application to patient-centered pharmaceutical care
- Practice continuing professional development as required by Regulated Health Professions Act
- General knowledge of appropriate dosages, delivery techniques, formulations and routes of administration of all medications.
- Knowledge & interpretation of laboratory and clinical parameters and their application to patient assessment & monitoring
- Knowledge & utilization of credible electronic & print form medical information resources
- General knowledge of aseptic technique & extemporaneous preparation of pharmaceuticals
- Basic computer skills, and knowledge of Microsoft applications
- Understanding and interpretation of research methodology and outcomes & critical appraisal of study design
- Excellent time management and organizational skills
- Ability to function as an independent practitioner
- Sound problem solving, communications (oral and written in the English language), conflict resolution and decision-making skills
- Ability to communicate in additional language(s) is considered an asset.
- Highly motivated team player with the ability to collaborate across disciplines/departments/programs
- Ability to perform multiple tasks and work under pressure
- Contribute to a strong teamwork environment
- Excellent attendance and punctuality

Hours of Work: Days, evenings and weekends. Please be reminded that as a Hospital, in order to provide the necessary patient care, our operations run 24/7. Therefore, depending on the program for which you are applying, it may be an expectation that you are available to work weekdays and weekends, as well as all shifts - days, evenings, and nights.
Wage: $56.41 - $66.99 per hour Shift Premiums:
- Evening: additional $2.26/hour
- Night: additional $2.98/hour
- Weekend: additional $3.14/hour
