Job summary

As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with intellectual disabilities and their families, including residential and day supports, employment assistance, community support, early childhood services, and respite care.

QUALIFICATIONS:
  • A high school diploma, 6 months of related work experience, and knowledge of healthcare procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment are required.
  • Post-secondary education focusing on Social Services or Developmental Services may be considered as an equivalent with no related work experience required.
  • Successful completion of internal legislated and mandated training is required.
  • A valid driver’s license is an asset.
JOB SUMMARY:

Contributes to the delivery of supports and programs aligned with individuals' life plans to meet their needs and enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:
  • Under the direction of a program supervisor, carry out assigned duties related to the provision of services and supports.
  • Support assessment of daily needs and assist in the implementation of individual life plans, monitoring personal outcomes.
  • Provide appropriate personal and physical care, including medication administration and crisis response.
  • Liaise with friends and family as needed to address needs and share information on behalf of individuals supported.
  • Support life skills training and provide emotional support to individuals on a one-on-one or group basis.
