As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with intellectual disabilities and their families, including residential and day supports, employment assistance, community supports, early childhood services, and respite care.

QUALIFICATIONS:
  • A high-school diploma, 6 months of related work experience, and knowledge of healthcare procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment are required.
  • Post-secondary education with a focus on Social Services or Developmental Services may be considered as an equivalent, with no related work experience required.
  • Successful completion of internal legislated and mandated training is required.
  • A valid driver’s license is an asset.
JOB SUMMARY:

Contributes to the delivery of supports and programs aligned with individual life plans to meet the needs of clients and enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:
  • Under the supervision of a program coordinator, responsible for executing assigned duties related to service provision.
  • Supporting assessments of daily needs and assisting in implementing and monitoring individual life plans to achieve personal outcomes.
  • Providing personal and physical care, including medication administration and crisis response.
  • Liaising with family and friends as needed to address support requirements and sharing relevant information.
  • Supporting life skills development and offering emotional support to individuals either individually or in groups.
