Job summary

HOURS OF WORK: 37.5 hours per week

As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with intellectual disabilities, as well as support to their families, including residential and day support, assistance with employment, community support, early childhood services, and respite.

QUALIFICATIONS:

  • A high school diploma, 6 months of related work experience, and knowledge of health-care procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment are required.
  • Post-secondary education with a focus on Social Services or Developmental Services may be considered an equivalent with no related work experience required.
  • Successful completion of internal legislated and mandated training is required.
  • A valid driver’s license is an asset.

JOB SUMMARY:

Contributes to the delivery of supports and programs consistent with life plans to meet the needs of individuals and enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:

  • Under the direction of a program supervisor, responsible for:• Carrying out regularly assigned duties related to the provision of services and supports.
  • Supporting assessment of daily needs of individuals and implementing life plans, monitoring achievement of personal outcomes.
  • Providing appropriate personal and physical care, including medication administration and crisis response.
  • Liaising with friends and family as required to address needs and sharing information on behalf of individuals supported.
  • Supporting life skills training and providing emotional support to individuals on a one-on-one or group basis.
