As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with intellectual disabilities and their families, including residential and day supports, assistance with employment, community support, early childhood services, and respite.

QUALIFICATIONS:
  • High school diploma, 6 months of related work experience, and knowledge of health-care procedures and mechanical equipment (wheelchairs, lifts, G-tubes, etc.) are required.
  • Post-secondary education focusing on Social Services or Developmental Services may be considered equivalent with no related experience.
  • Successful completion of mandated training is required.
  • Valid driver’s license is an asset.
JOB SUMMARY:

Contributes to the delivery of supports and programs aligned with individual life plans to meet their needs and enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:
  • Under the supervision of a program coordinator, perform duties related to service delivery and support.
  • Support the assessment of daily needs and assist in implementing life plans, monitoring personal outcomes.
  • Provide personal and physical care, including medication administration and crisis response.
  • Communicate with friends and family as needed, sharing relevant information.
  • Support life skills training and offer emotional support to individuals, either one-on-one or in groups.
