Hours of Work:

Up to 25 hours per week - 24/7 operation (shifts to include weekdays, afternoons, overnights, and every weekend).

It supports over 6,000 individuals with intellectual disabilities and their families through residential and day support, employment assistance, community support, early childhood services, and respite care.

Qualifications:
  • High school diploma, 6 months of related work experience, and knowledge of healthcare procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment.
  • Post-secondary education in Social Services or Developmental Services may be considered as an equivalent with no related experience required.
  • Successful completion of internal legislated and mandated training.
  • Valid driver’s license is an asset.
Job Summary:

Contributes to the delivery of services and supports aligned with Person Directed Plans, Passport funding budgets, and Ontario Passport Program Guidelines to promote community participation, social inclusion, skill development, and independence.

Responsibilities:
  • Under the supervision, carry out duties related to providing services and supports.
  • Support assessment of individual needs, implementation of person-directed plans, and outcome monitoring.
  • Provide appropriate care, including crisis response.
  • Liaise with support networks and families as needed to address needs and share information.
  • Support community participation, skill development, social inclusion, and offer emotional support to individuals supported.
