As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with intellectual disabilities and their families, including residential and day supports, employment assistance, community support, early childhood services, and respite care.

QUALIFICATIONS:
  • A high school diploma, 6 months of related work experience, and knowledge of health-care procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment are required.
  • Post-secondary education focusing on Social Services or Developmental Services may be considered equivalent with no related work experience required.
  • Successful completion of internal legislated and mandated training is required.
  • A valid driver’s license is an asset.
JOB SUMMARY:

Contributes to the delivery of supports and programs aligned with individual life plans to meet their needs and enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:
  • Under the direction of a program supervisor, responsible for carrying out assigned duties related to service provision.
  • Supporting assessment of daily needs and the implementation and monitoring of individual life plans and personal outcomes.
  • Providing personal and physical care, including medication administration and crisis response.
  • Liaising with friends and family as needed to address support needs and sharing information on behalf of individuals supported.
  • Supporting life skills training and providing emotional support to individuals on a one-on-one or group basis.
