As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals with an intellectual disability, along with support to their families, including residential and day support, assistance with employment, community support, early childhood services, and respite.

Qualifications:
  • Post-secondary education with a focus on Social Services or Developmental Services is required, along with knowledge of health-care procedures and mechanical equipment such as wheelchairs, lifts, G-tubes, and other specialized equipment.
  • Post-secondary education in a non-related field requires a minimum of 1 year of related work experience.
  • Successful completion of internal legislated and mandated training is required.
  • Valid driver’s license is an asset.
Job Summary:

Identifies the needs of individuals supported and delivers supports and programs consistent with their life plans to enhance their capacity to function in all aspects of community living.

Responsibilities:
  • Under the direction of a program supervisor, responsible for identifying program needs, developing and implementing life plans, and monitoring achievement of personal outcomes.
  • Providing case management, personal and physical care including medication administration, crisis response, advocacy, and liaising with family, medical, and other professionals.
  • Devising and providing life skills training and emotional support, referring to professionals when necessary.
  • Maintaining documentation, monitoring finances, ensuring an appropriate standard of living, and assisting with housing placement.
  • Providing direction to part-time staff as required.
