Job Summary:

The Manager Home and Community Care (HCC) - Registered Nurse provides professional and practice leadership to staff and is responsible for ensuring the provision of safe, effective quality client care within their program area. The position is accountable for working as part of an integrated Regional management team to ensure the successful execution of site operational goals and for managing the delivery of in-home care services, staff hiring, supervision and training, client service programs, and team operations.

Key Responsibilities:

  1. Directs and monitors delivery of client care services and day-to-day functions and performance of front-line staff in accordance with current regulatory guidelines and VON policies, procedures, and standards.
  2. Assists staff to complete risk assessments, investigations of issues, injuries, and reporting related to client or employee incidents.
  3. Monitors performance related to funder expectations regularly and contributes to the development and implementation of quality improvement activities to achieve performance targets through monitoring of reported services.
  4. Guides new employees through the orientation process and ensures all probationary reviews and stay interviews are completed.
  5. Applies a working knowledge of the relevant collective agreements and application of local labour rules in daily decision-making activities.
  6. Participates in budget preparation, financial performance management, and measurement reports.
  7. Participates in local, regional, or national teams or committees as required such as Quality Care and Service, Joint Occupational Health and Safety, Accreditation, Recruitment, and Retention.
  8. Develops team procedures and guidelines in collaboration with internal stakeholders to ensure best practices in task and program completion.
  9. Builds and sustains strategic relationships with current, new, and prospective VON partners and community organizations.
  10. Handles employee, family, and client complaints or issues related to service delivery, engaging other VON managers in resolution and escalating if necessary.
  11. Participates in the on-call rotation.
  12. Ensures occupational health and safety policies, programs, and activities are performed by staff in compliance with VON policies and the Occupational Health and Safety Act.
  13. Assesses community needs and assists in developing business and funding proposals for emerging opportunities.
  14. Supports department presentations and reports.
  15. Completes internal and external reports, conducting relevant analysis and ensuring timely submission.
  16. Performs risk assessments and investigates issues, incidents, or injuries as required.
  17. Facilitates staff development and growth through team approaches, mentorship, and collaboration with practice leaders, educators, and clinical experts.
  18. Provides clinical advice, support, and reference resources to service providers.
  19. Assists with the delivery of in-home care services as needed.

  10. Ensures a safe, healthy workplace in line with occupational health and safety regulations.
  11. Supports work/life balance and professional development.
  12. Encourages the use of technology for knowledge management.
  13. Maintains confidentiality and safeguards personal information.

External and Internal Relationships:

  1. Communicates proactively with home care teams and management.
  2. Develops cross-departmental relationships to meet objectives.
  3. Interacts with external organizations such as LHIN, CCAC, and community health agencies.

Education, Designations, and Experience:

  • Post-secondary education in a related field.
  • Registered Nurse with current certification.
  • At least 5 years of experience in the relevant functional area.
  • Minimum of 2 years of supervisory experience.
  • Experience in community health and collaboration with community partners.
  • Experience with negotiations, conflict management, financial management, and working with staff and volunteers.
  • Experience in a unionized environment is an asset.
