About the Role :

The Community Wellness (CW) program supports holistic approaches to promote healthy living, safe communities and the prevention of family violence. The CW program delivers a variety of client participant-based services to individuals, families and children with the primary focus to address their health, healing and wellness concerns. Programming for people of all ages, to ensure culturally vibrant and safe way of life is achievable for everyone. The CW Coordinators assist individuals to identify concerns and develop action plans to address goals, barriers and resolution.

What you will be doing :

  • Ensure that Community Wellness program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose
  • Facilitate client services such as home visiting, service support assessments, referrals, developing a wellness plan, and attend consultations with client.
  • Coordinate traditional based activities which involve the specific use or development of traditional Métis knowledge, practices, teachings or services to achieve the objectives of the activity or event.
  • Provide education / informing / learning through organized, facilitated or coordinated activities. Participant based activities are group rather than individual activities that are open to a targeted group of people within the community or the community at large.
  • Provide education / information related to health and well-being and activities to reduce isolation.
  • Determining courses of action for client-driven needs.
  • Record client services in the Online Health and Access Tracking System (OHATS).
  • Maintain confidentiality at all times, and adhere to privacy legislation
  • Perform Administrative duties, and other duties as instructed by the Program Supervisor
  • Financial reporting specific to your program site and budget
  • Reflect Métis cultural approaches as a part of the activities and services
  • Participate in on-going professional development activities as time and financial resources permit
  • Attend training and meetings outside Métis Nation of Ontario working hours
  • Fulfill all other responsibilities as determined by management of Community Wellbeing Branch

Qualifications :

  • Degree or diploma in social services
  • 1 or more years’ experience in client case management
  • 1 or more years’ experience hosting workshops or activities for community
  • Proficiency with computer programs (Outlook, Excel, Work and PowerPoint)

Requirements :

  • Knowledge of the Métis Nation of Ontario and programs offered
  • Knowledge of Indigenous programs and services
  • Ability to work as part of a team and independently with minimal supervision
  • Well-developed administrative, organizational and communication skills in order to meet deadlines
  • Ability to lift 11.5kg when required
  • Valid driver’s license and access to a reliable, insured vehicle are required
  • Bilingualism would be considered an asset
