Job description

Accountabilities :

  • Work collaboratively with the Pharmacy Team to provide patient-centred care.
  • Perform dispensary duties including medication order entry and medication profile review.
  • Provide drug information to physicians, nurses, and other clinicians to enhance patient care.
  • Verify all medication orders for inpatients.
  • Provide therapeutic drug monitoring.
  • Participate on hospital committees as required.
  • Perform and/or supervise in-patient dispensing, including computerized order entry, unit dose, centralized IV admixture services, TPN, chemotherapy, and understanding of automated systems, drug information, and administrative activities as assigned.
  • Resolve day-to-day clinical issues.
  • Assist with the implementation of pharmacy initiatives to improve patient care and the efficiency and effectiveness of pharmacy operations.
  • Perform other duties as assigned.

Qualifications :

  • Currently licensed (or eligible) by the Ontario College of Pharmacists, Part A of the registry.
  • Relevant clinical experience preferred.
  • Excellent attendance, punctuality, and work record.
  • May be required to travel between and work at all Osler sites.
  • Excellent computer skills, including Meditech experience preferred.
  • Excellent oral, interpersonal, and written communication skills.
  • Demonstrate initiative and a willingness to work collaboratively in a team environment.
  • Strong clinical assessment skills in identifying and resolving drug-related problems.
  • Ability to apply clinical judgment in analyzing and solving clinical problems.
  • Must be able to meet the physical demands of the position.

Additional Information :

  • Hours : Nights, including weekends and statutory holidays (subject to change based on operational needs).
  • Salary : Minimum $50.51/hour, Maximum $64.54/hour.
