We have an exciting opportunity to join our Primary and Preventative Health team in the Discharge Support Service department in a Casual capacity. Discharge Support Coordination is a vital role working across acute and community services.
The aim of the role is to facilitate a smooth transition for clients and carers referred to the Discharge Support Service with complex discharge needs, from an acute episode of ill health back to the community.
The discharge support care coordinators facilitate appropriate support services for clients referred to the Discharge Support Service and act as a broker in purchasing those services for clients requiring Short Term Support services.
This role requires effective teamwork with multidisciplinary team members and external service providers.
To be successful, candidates must have or obtain a National Criminal Police Check. Depending on the role, a Working with Children Check Clearance, Disability Worker Exclusion Scheme check, and/or NDIS Worker Screening Check may also be required. All employees must comply with WDHS vaccination requirements.
